Description of Our Memory Care Program

In continuing with the mission, Villa St. Francis has established Memory Care in Milwaukee services to meet the needs of those residents who suffer from Dementia.  Our memory care is located on the 3rd floor of the facility.  We specialize in memory care programming to meet the needs of up to 24 residents.  All apartments are private.

 

Milwaukee Memory Care Program Goals

The overall goal of Villa St. Francis Memory Care Program is to maximize the resident’s cognitive psychosocial, and functional skills in order to help maintain their highest level of functioning.  Villa St. Francis Memory Care will provide a supportive community setting and personal care services to persons with irreversible cognitive loss in a clean, comfortable, secure environment.

This will be accomplished through attitudes of hopefulness, in a structured yet warm environment.  Each resident will have individual treatment plans that have realistic goals which will include input from the Villa St. Francis team, families/significant others, and case management teams. Residents will have the opportunity for social interaction in supervised groups.

 

Community Integration for Memory Care in Milwaukee

Imperative to the program is the emphasis on community integration for each resident.  Social activities, religious activities, cultural events, and interaction through community events and activities will be available.

Specific Program goals include:

  • Assisting and training staff in personal care activities such as dressing, grooming, and bathing.
  • Assisting residents in appropriate socialization, including productive and independent leisure time, one on one interaction, and group activities.
  • Assisting residents in understanding and expressing their emotions and feelings, focusing on building good self-esteem and a positive sense of self-worth.
  • Encouraging and helping residents in maintaining involvement with their families and other significant people in their lives.
  • Accurately assessing and responding to the unique and changing needs of each resident.

 

Milwaukee Memory Care Program Services

Program services are provided as described in each resident individual service plan in the following areas:

  • Health monitoring/psychiatric monitoring are provided by licensed nurses who ensure that each resident receives routine health screening and yearly physician visits. Villa St Francis works with a number of health care providers, dentists, specialists, and pharmacies.
  • Medication monitoring is done routinely by Villa staff and quarterly through our pharmacist. Medication administration is done by authorized staff when ordered by a physician.
  • Leisure Services: Designed to maximize community involvement and keep those residents with memory loss as active as possible. Villa St. Francis participates in 2 programs specific to dementia:  Memories in the Making which uses artistic expression.  And Music and memories which uses music to help residents express themselves.
  • Nutrition: 3 meals are served in the 3rd floor dining room daily.  Multiple finger foods and snacks are made available 24 hours a day for those who cannot sit for meals.  Hydration station is available 24 hours a day to ensure adequate hydration.
  • Personal care services are provided by Villa staff who are trained to focus on patience and reattempts for those who struggle completing activities of daily living.

Our Dementia Difference Program

Education  Caregivers working in Angela’s Crossing have been trained specifically in dementia care and receive on-going training.  We believe staff trained specifically in dementia care are able to provide better quality of life for residents.

AboutME Like all individuals, people diagnosed with dementia are whole people with their own values, preferences and personal life stories.  AboutME is a resource for people with dementia to provide wayfinding, to tell care staff about themselves, their history, needs, likes, interests, etc. to build relationships with staff, residents, visitors and families.  

Dining – To provide a dining experience that promotes independence and increased food intake.  Our residents enjoy warm moist towels before beginning their dining experience, which is both soothing and sanitizing.  Angela’s Crossing offers a Fresh Bites program (finger food) for those residents who have difficulty or who have forgotten how to use utensils.  

Music & Memories – The program helps people with dementia and Alzheimer’s find renewed meaning and connection in their lives through personalized music.  The connection of music and memory is one of the areas of the brain that remains intact with many people living with the disease. We have personally witnessed the magic of music.  Residents who are less verbal will sing along to the songs.  

Memories in the Making ™ – The Alzheimer’s Association “Memories in the Making” program offers creative art expressions for individuals with mild cognitive impairment.  “Memories in the Making” participants create imaginative and colorful water coloring paintings. Most have no prior art experience. The painting process provides an opportunity to engage socially and reminisce about meaningful life moments and individual or personal interests.  A trained facilitator documents these memories, rekindling communications with family, friends and caregivers. The art our residents with dementia create is truly amazing. One of our family members commented “Whenever I see my Mom’s artwork, it brings tears to my eyes….in a good way!” 

Aroma Therapy – Evidence supports that using certain types of oils can stimulate appetite, be relaxing and calming.  There are other therapeutic advantages.  

PHOTAVIA™ – Develops therapeutic tools on digital screens that engage, educate and create experiences.  

Massage TherapyAs little as five minutes of hand massage has been found to reduce cortisol levels and produce a physiological relaxation response. Gently pressing the palm and rubbing the knuckles in therapeutic motions has helped dementia patients reduce their agitated behavior during morning care routines. Hand massaging not only has therapeutic value but also provides an opportunity for social interaction.  

Pet Assistance Therapy –All Companion Pets as well as babies are provided to foster meaningful connections through plan, joy and happiness.  The impact of robotic companion pets can provide comfort to those with dementia.  

Support GroupAngela’s Crossing Support Group, led by our Social Worker and Chaplain, provides meaningful time for family caregivers to focus on their own feelings and needs.  Support Group goals allow participants to talk openly and honestly about their lives as caregivers, their feelings and the impact Alzheimer’s Disease/dementia is having on their loved one, their family and their lives. In so doing, the hope is that they gain a sense of empowerment and control, improve coping skills and sense of adjustment, and reduce distress, depression, anxiety and fatigue in their lives and feel less lonely and isolated by building relationships with other people experiencing similar situations.
